Your First Appointment

 

Thank you for choosing the Allergy and Asthma Center of Minnesota. It is our goal to provide high-quality and cost-effective care to everyone. We strive to make everyone happy that they chose us to take care of their health. We do this by providing friendly care and working until we find out what is causing your unwanted conditions and helping eliminate or mitigate your symptoms.

 

Please plan to spend at least 60 minutes with us for your initial visit and arrive 10-15 minutes before your scheduled time. Please bring the following to your first appointment.

 

• Driver's license

• Your insurance cards

• Physician referral forms, if required by insurance

• Information about your medical and surgical history

• Recent x-rays or relevant records

• List of prescriptions and/or over-the-counter medication, including dose and frequency
